CCMS-AAEM's Discussion on Agitated Patient

This month we reviewed Chan et al’s paper from E Clinical Medicine “Intramuscular Midazolam, Olanzapine, or Haloperidol for the Management of Acute Agitation: A Multi-centre, Double- Blind, Randomized Control Trial.”  This was a multicenter RCT of 206 patients randomized to either 5 mg IM Midazolam, 5 mg IM Olanzapine, or 5 mg IM Haloperidol. Midazolam achieved adequate sedation the fastest (8.5 min), and at 60 minutes greater than 85% of patients were adequately sedated.  There were minimal adverse events.


CCMS-AAEM's Discussion on Etomidate Vs. Ketamine

The ensuing discussion had several points of agreement, most notably etomidate overall is a more hemodynamically stable agent, this is based on both the literature and personal experience of Ketamine being a less predictable agent. However, that if the concern regarding Ketamine is hypotension, that in the era of resuscitate before you intubate vasopressors should be in hand to both prevent and manage perintubation hypotension.
